You can pay Yelp to "clean up" your reviews. Also there's tons of sites that will do fake reviews. But ya, a bunch of 4 star reviews is usually a decent sign. If I'm in a new town or neighborhood and looking for food I usually just ask someone that doesn't look like a total scumbag what the best food is.
