Medical Beat: What are T/E ratios? And why do cutoff limits vary?

Following UFC 146 headliner Alistair Overeem's failed drug test, my inbox is full of questions regarding T/E ratios and the like.

So maybe it's time for a quick physiology primer for you fight junkies.

Testosterone (T) is the naturally occurring male hormone produced primarily in the testes. Epitestosterone (E) is an inactive form of testosterone that may serve as a storage substance or precursor that gets converted to active T.

Most men have a ratio of T to E of 1:1, which means normal men have equal amounts of T and E in their blood. There is some normal ethnic and time of day variation in the normal T/E ratio (as low as 0.7:1 and as high as 1.3:1).

Statistics reveal that a ratio of up to 3.7:1 will capture 95 percent of all normal men, and a ratio of up to 5:1 will capture greater than 99 percent of all men. That's why the World Anti-Doping Agency (WADA) allows up to 4:1 (so its test is at least 95 percent accurate) and the Nevada State Athletic Commission, the NCAA and some others allow up to 6:1 (for 99 percent accuracy). The whole goal is to not label someone a cheater when he or she isn't. (Very, very rarely, some people are just freakishly high, but they have a ratio of less than 6:1).

Overeem, of course, had an eye-poppin' 14:1 ratio.

T/E ratios are used in performance-enhancing-drug (PED) monitoring because taking an external (exogenous) source of T will not effect the E levels in the blood. E stays the same, but T climbs because of the injection, and the T/E ratio follows suit and climbs.

The half-life of injectable T is only eight days. So every eight days, half of the T you took is washed out of your blood. Therefore, if a cheater knows when he is likely to be tested (i.e. post-fight), he doesn't need to be a genius to know when to stop taking ("cycle off") to test lower than 6:1 or 4:1. Twenty-four days is three half-lives, and virtually none of the extra T is left in your system to get you busted.

T abuse is making a resurgence because of therapeutic-use exemptions (TUE for TRT). Also, it naturally occurs in men's blood – unlike other anabolic steroids that at any level are unnatural (not made within the body) and must be masked in an attempt to beat the test. Cheaters don't have to mask T, so they don't worry about testing positive for a masking agent. They just need enough time for their bodies to get rid of it naturally.

If you don't know when the test is coming, you cannot adequately plan or time when to stop taking a PED like T. This, of course, is the rationale for random testing.

There you have it.

And remember, a little knowledge is a dangerous thing!

Junior dos Santos Proclaims Drug-Free Status Following Overeem's Failed Drug Test

Apr 5, 2012 - A day after news broke that Alistair Overeem had failed a pre-UFC 146 drug test, there are still many more questions than answers.

Overeem and his camp have yet to say anything, and it is still unclear whether the UFC will have to book a new main event for its Memorial Day weekend show.

However, UFC heavyweight champion Junior dos Santos, the man Overeem is, at least as of Thursday afternoon, still scheduled to fight on May 26, broke his silence on Twitter to proclaim his drug-free status.

"I am professional and I'm champion without ever having used anything for better performance when you guys see me fighting that's really me," dos Santos wrote Thursday afternoon.

JDS added that, much like the rest of the MMA community, he is unclear what's next for him.

"About what happened to Overrem I don't know anything more than you guys know and for me It's between him and the Nevada Athletic Commission.

"I know a lot about me that's it!"

Keith Kizer, the Nevada State Athletic Commission executive director, told MMAFighting.com on Thursday that Overeem had a testosterone-to-epitestosterone ratio of 14:1 in his failed urine test, well above the the state's 6:1 limit.

To be honest, i would be more surprised if the nigga passed the drug test. He's just as bad as mir. Both them mothers got big, so quick, they got muscles growth on their face, lol muthafucka how does someone do that naturally? Fucking cheeks are all spasming and shit.

what i dont get is dana saying he understood chael's testosterone level and it wasnt that big of a deal but then outraged about this one? isnt it possible that maybe overrem just produces more testosterone then the average man lol? i mean i think he was juicin but i think he should test his 2nd urine and hope for the best. juice or not i wanna see this damn fight BADLY.

Your epi generally follows your natural test. A standard T/E ratio is 1:1 for an older male.
14:1, especially at his age, he's definitely getting an external substance.
Sonnen had more and admitted to injecting but claimed to have a script.
Floyd Landis had 11:1

Even if someone has a script and goes on therapy (200mg per week), at most they could bring their level up to is about 6:1 which is the max. He was at 14:1, which more of less seems like he was doing around 500mg per week, at the very least. The T/E ratio is meant to prove you have an external source.

If they were to test his testosterone levels, sure he could genetically have higher levels but his T/E ratio shouldn't increase. When you inject testosterone, your E levels do not increase. Only your T levels.

UFC files license application for UFC 146's Alistair Overeem, NSAC reviews April 24

The UFC has not given up on Alistair Overeem.

On Friday afternoon, UFC officials filed a license application on the heavyweight contender's behalf for a bout with Junior Dos Santos at UFC 146, Nevada State Athletic Commission Executive Director Keith Kizer told MMAjunkie.com (www.mmajunkie.com).

Overeem has not contacted the commission since he failed a pre-fight drug test for the pay-per-view event, which takes place May 26 at MGM Grand Garden Arena in Las Vegas.

Additionally, Overeem has not requested the "B" sample of the test that flagged him for elevated levels of testosterone. He is expected to be present at an NSAC meeting in Las Vegas on April 24, when the NSAC will review his application. theflyingkneemma.com initially reported the license application.

Because Overeem is not currently licensed, he does not face an immediate suspension and/or fine. However, the NSAC could deny his license or sanction him in other ways.

"When you apply for a license, anything and everything can be brought up," Kizer said.

Prior to the application, NSAC chairman Raymond "Skip" Avansino Jr. asked Overeem to appear before the commission at the meeting, likely to answer for the failed drug test that served as part of a conditional license granted to fight Brock Lesnar at UFC 141.

After missing a pre-fight drug test for that December event, Overeem's appearance was contingent on undergoing additional drug testing. During an NSAC hearing in December, he said he had missed the test while flying to Holland to care for his ailing mother. Concerns over the missed drug test and efforts to get him tested in his native country led the commission to issue a conditional license in which he was required to submit to two pre-fight tests, a fight-night test, and two random tests within six months of the fight. The license expired on Dec. 31.

The UFC 146 pre-fight test, which was conducted after a UFC 146 press conference on March 27, served as one of Overeem's two random tests. This past week, his "A" sample was flagged for a testosterone-to-epitestosterone (T/E) ratio of 14:1, well over the NSAC's 6:1 limit. The former Strikeforce champion was among six top-billed fighters who were tested for the all-heavyweight PPV card. All other fighters tested clean for performance-enhancing drugs and drugs of abuse.

Overeem could ask that his "B" sample – a second portion of urine taken from the UFC 146 test – be tested by the lab that found the result or by a separate accredited facility. He has yet to do so.

Overeem had never tested positive for banned substances before this past week's result. He has yet to comment on his failed test.

Over the weekend, UFC president Dana White said a co-main event fight between former champ Frank Mir and Cain Velasquez will go on as planned, ending, for now, speculation that Mir would be installed as Overeem's replacement.

I forget the legality of it but there's a chance he could lawyer-up and say that since he was tested without being licensed he can't be held accountable for it and get away with it.

Yeah from what i'm reading what it boils down to is a fighter is only susceptible to punishment for banned substances while they are actively licensed. Even though he agreed to 2 random tests to be conditionally licensed for the Brock fight his legal team could still use the "so you randomly tested me while not being licensed you still can't punish me" argument and now that he has officially applied he must NOW be able to pass.

I think Reem will fight.

Not really a pro but thanks. Anyone who has been cycling for a long time will be able to tell you these things. I'm not a chemist or doctor. I just know a lot about gear, because I use as well.

I have no clue if he can get it, but I hope so. He didn't get caught using, but still, look at Manny Ramirez. He got caught using a substance used to "cycle off", in the off season. His test levels were fine, but he had HCG in his system which is used to kick start your natural production after getting off. He got suspended for 50 games just for that. We'll see how strict they are on Overeem. At the very least, he has to cycle off. If it's just testosterone that he's on, it depends on the ether that he's running. It could take anywhere from a week to a couple months. Just depends on the half life of the chem he used.

i have a question for people who juice

is there any benefits to HGH or test for someone lookin to gain 20+ lbs muscle mass, but has trouble putting on weight? consider the fact the person is diabetic and has already gone from over 350lbs nearly solid fat lol down to about 125 skin and bones

is there any negative side effects that could be fatal?

just askin here cuz you guys seem to know alot about this shit
