it depends on the species and on the selective pressure and the development of the reproductive organs
as far as mammals are concerned, the initial form had two penises and two vagina and uteri, two of everything (what we see today in Marsupials)
the trend was towards mergeing of the extenal genitalia and the uteri, the extreme case is observed in humans, there are a number of transitional states observed in lower Placentalia
I don't have an explanation why exactly these trends are observed, probably having two of everything wasn't the most practical thing so it was selected against
