not sure , on the industry standard thing, but when I want to give some overall volume, I slap a waves L2 on the master track and adjust accordingly between master fader volume and L2 threshold.
Sometimes isn't the best thing for the mix but seems to work out nice for a quick fix.

It depends on what you plan to do. Are you trying to master your music yourself or are you talking your mixes to someone else to do it for example. You haven't provided enough information. And what exactly are you trying to accomplish? If you're just burning cds to play in your car then turn the volume up as much as you want.

There is no set standard. The more room you leave to play, the more you can play. The less room you leave to play, the less you can do.

the -12 thing is just digitals conversian to analog nothing to worry about its a math formula. In that right all you really have to know there is digital distortion sounds really bad. one trick is to get some sort of peak limiter plug in and hook it to the master fader in protools -1 so you never get digital distortion and you can puch your mono tracks to the volume you want
