looking for a cd single template

im looking for a cd single template that is a lil bigger than an insert and is made of cardboard and is sealed on 3 sides and one side is left open to access the cd...there are no flaps or folds to open up...

there are no slim or regular jewel cases...

this is the closest ive seen but i dont think this is it...

does anyone know what the name of it is or where i can find the template...thanks...

The PP-1000

I see how it works by the Crop Marks, I just don't understand the square in the middle, it throws the rest of the Template off.....
