They can but they get thousands and thousands of these. It appears they only randomly audit a few because I got two checks when they said they looked them up in relation to the number provided and well, yeah.
Worse case scenario is they ask for more info or decline you.

Not to be an ass breh but it tells you right there on the second page of the claim form how many you can do. Some of you guys need to actually read them before asking questions, they aren't hard to figure out.
But to answer your question it's three at $5 a piece for $15 total and if money is left over after all claims are filed they will possibly triple it up to $45.

I feel you breh and a few can be kinda tricky. I've had to read a handful of them over two or three times to get what they were saying, and apparently seeing I've been on the lower end of two of them even I messed up a couple times. But quite a few people in here are asking very obvious questions or other questions that have been answered numerous times where if they did the reading themselves they would have known the answer in a few minutes.
Like I said I didn't mean any of that last part at you personally because I haven't seen you ask before but it's just getting a little tiring when it's pretty easy to figure out if people would take the time to read it first.

My printers busted. Can't scan it but I had to fill out a doctors form a few months ago and you can add text/cursive writing (for the signature just click "Place initials" and sign it, place it on the signature line and be done) to a pdf document using Adobe Reader 11.0. I just did it with the contact one and when you send the email you just "add attachment".
Or you can go to some fedex spot and pay to have it done using their equipment but fuck that. IDK if it needs to be sent BY or ON the 14th at the latest so for me it;s tonight or not at all.
